Home Medicines Review (HMR)
An HMR is a comprehensive, structured review of all the medicines a patient takes, including prescription, over-the-counter and complementary medicines. Following a GP referral, our pharmacist visits the patient at home, reviews their medicines with them and sends a detailed report back to the GP.
