Who we are
Better medication management through collaboration
MedReviewsMMR is a pharmacist-led medication review service. We conduct Home Medicines Reviews for patients referred by their GP, sitting down with them in their own home to understand how they actually take their medicines day to day.
Many medication problems only become visible at the kitchen table: duplicated brands, old scripts still in use, confusion about timing, or side effects the patient hasn’t mentioned. Our job is to find those problems and give the GP clear, practical recommendations to act on.
We speak English, Urdu, Hindi and Punjabi, which helps patients from Melbourne’s South Asian communities talk openly about their medicines.
