Information for GPs
Everything you need to refer a patient for a Home Medicines Review and claim MBS Item 900.
How it works
Three steps from referral to report
- Identify and refer. Identify an eligible patient and send us a referral. Record the referral in the patient’s chart.
- We arrange the review. We contact the patient to book a home visit at a convenient time, at no cost to them.
- You receive a report. We send a comprehensive report with clear recommendations. You discuss it with the patient and agree on a medication management plan.
