MBS item 900: claiming a Home Medicines Review

What a GP needs to do to claim item 900, how often it can be claimed, and what changes in July 2027.

At a glance

What item 900 pays for

MBS item 900 is the GP’s part of a Domiciliary Medication Management Review (DMMR), known to patients as a Home Medicines Review. The pharmacist’s review is funded separately through the PPA HMR Program, so the patient pays nothing for it if eligible.

Steps to claim

  1. Assess the patient. They live in the community, have a chronic medical condition or a complex medication regimen, and are not meeting their therapeutic goals. Get their consent.
  2. Refer to a community pharmacy or an accredited pharmacist (that can be us), with the clinical information the review needs.
  3. Discuss the findings with the reviewing pharmacist, including the suggested medication management strategies. This can be in writing, by phone or by video.
  4. See the patient in person, talk through the findings and write a medication management plan with them.
  5. Give the plan to the community pharmacy the patient chooses, then claim item 900.

Limits

  • Once per patient in a 12-month period.
  • Not in the same 12 months as item 245.
  • Exception: a significant change in the patient’s condition or medication regimen that needs a new review.
  • Not for hospital inpatients or people living in residential aged care.

The pharmacist’s HMR service has its own limit under the PPA program rules: generally once every 24 months, sooner if clinically necessary (for example after a significant change), plus up to two follow-up services within 9 months of the first interview. PPA HMR program.

Coming change: 1 July 2027

From 1 July 2027, patients will need a GP chronic condition management plan put in place or reviewed in the previous 18 months to access a DMMR. Check that your patients’ plans are current before referring.
