Ask your GP about a Home Medicines Review

Make a short letter to take to your doctor. It takes two minutes and nothing is sent to us.

For patients and carers

A Home Medicines Review starts with your GP

We can only visit after your doctor refers you. If you think a review could help, fill in the form to make a short letter, print it and take it to your next appointment. Your GP will decide whether a review is right for you.

  • Reviews are free for eligible patients with a Medicare or DVA card.
  • A pharmacist visits you at home and goes through all your medicines with you.
  • We can speak with you in English, Urdu, Hindi or Punjabi.

This form does not send anything to us. It makes a letter on your screen that you can print and take to your doctor.
