When to refer for a Home Medicines Review

A quick guide for GPs and practice nurses: who benefits most, how to refer, and what happens next.

Who benefits most

Patients most likely to benefit from an HMR

These are common signs that a patient’s medicines need a closer look. Funding eligibility is set by the PPA HMR Program Rules: the patient must live in the community, hold or be eligible for a Medicare or DVA card, and have a clinical need confirmed by you.

Their medicines

  • 5 or more regular medicines, or more than 12 doses a day
  • Significant changes to the regimen in the last 3 months
  • High-risk medicines such as anticoagulants, opioids, insulin or benzodiazepines
  • Medicines that need monitoring or have a narrow therapeutic index
  • Several prescribers or pharmacies involved

Their circumstances

  • Recent discharge from hospital
  • Difficulty managing medicines: language, literacy, vision, dexterity or memory
  • Suspected non-adherence or confusion about what to take
  • Symptoms that may be medicine-related: falls, dizziness, confusion
  • Not responding to treatment as expected

How to refer

Five steps, most of them ours

  1. Discuss the review with your patient and get their consent.
  2. Send the referral: use the online form, or our referral letter template from your clinical software.
  3. Record the referral in the patient’s notes. You need this for MBS item 900.
  4. We do the rest: we contact the patient, visit them at home and send you a report by secure link.
  5. Discuss the report with your patient, agree on a medication management plan, and claim MBS item 900.

Eligible Patients

(Based on PPA HMR Program Rules)

A patient may be eligible for a funded HMR if they meet ALL of the following criteria:

  1. Hold a current Medicare or DVA card, or be eligible for a Medicare card.
  2. Live in a community setting (their own home or a non-residential setting).
  3. Are at risk of, or experiencing, medication misadventure.
  4. Have an identifiable clinical need for an HMR, confirmed by the referring medical practitioner.
